How to Escalate a Complaint

  1. Call 1-800-282-2881 first. Available 24/7. Ask for a supervisor immediately if the first agent cannot resolve the issue. Get a case number on every call and note the representative’s name.
  2. Request a formal appeal or coverage review. For prescription denials or prior authorization refusals, you have the right to formally appeal under federal law. Ask the rep to initiate the appeal on the first call — outcomes must be delivered in writing within federally mandated timelines.
  3. Submit a written complaint online. Log in at express-scripts.com and use the message center or contact form. Written submissions create a time-stamped formal record.
  4. Send a certified letter to HQ at Express Scripts, Inc., Attn: Patient Relations, 1 Express Way, St. Louis, MO 63121. Include your member ID, prescription details, prior case numbers, and the resolution requested.
  5. File with your State Insurance Commissioner. State-level regulators increasingly have direct PBM oversight authority for coverage decisions, billing, and network practices.
  6. File with CMS for Medicare Part D complaints at medicare.gov or by calling 1-800-MEDICARE (1-800-633-4227). CMS has direct regulatory authority over Medicare PBM operations.
  7. File with the BBB at bbb.org for a formal public record requiring a company response.
  8. File with the FTC at reportfraud.ftc.gov — the FTC has increased PBM oversight and handles deceptive billing and unfair practice complaints.
