About

Anthem, Inc. rebranded to Elevance Health, Inc. on June 28, 2022 — shares began trading on NYSE under the new ticker ELV, replacing ANTM. As of 2025, Elevance Health serves 45.2 million medical members, employs 97,100 people, and reported FY2025 operating revenue of $197.6 billion — up 13% from 2024 — making it Indiana’s largest publicly traded company.

How to Escalate a Complaint

  1. File a formal appeal / grievance via member services (number on your ID card). Get a case reference number. You have up to 180 days from a denial to file a Level 1 internal appeal in most plans.
  2. Request an Expedited Appeal for urgent/ongoing care. Federal rules require response to expedited internal appeals within 72 hours.
  3. Request External Independent Review after internal denial. ACA Section 2719 gives all non-grandfathered plan members the right to external independent review of claim denials.
  4. Send a certified letter to the Indianapolis HQ. Elevance Health, Inc., Attn: Member Appeals & Grievances, 220 Virginia Avenue, Indianapolis, IN 46204 — include member ID, plan name, claim reference, all prior case numbers, and specific resolution requested.
  5. File with your state Insurance Commissioner. Indiana residents: in.gov/idoi or (317) 232-2385. State insurance commissioners have direct enforcement authority over health insurers.
  6. File with the BBB under the Indianapolis, IN chapter.
  7. File with your state AG for deceptive coverage representations and systematic billing errors.
  8. File with CMS (Medicare/Medicaid plans) at medicare.gov or 1-800-MEDICARE (1-800-633-4227).
  9. File with the Dept. of Labor EBSA (employer ERISA plans) at dol.gov/agencies/ebsa or 1-866-444-3272. EBSA can investigate ERISA violations and compel compliance.
  10. Consult a health insurance attorney or patient advocate for high-value denials (surgery, oncology, specialty drugs) — many work on contingency for ERISA disputes.
