How to Escalate a Complaint

  1. Call your regional Member Services line and file a verbal grievance — get a formal grievance reference number.
  2. File a formal written grievance at healthy.kaiserpermanente.org/support/submit-a-complaint — triggers Kaiser’s regulatory compliance obligations under California law.
  3. Request an Independent Medical Review (IMR) — California only. If Kaiser denies a treatment as not medically necessary, California members can appeal to the DMHC for a free independent review. Kaiser must comply with IMR outcomes.
  4. File with the DMHC (California). The Department of Managed Health Care at dmhc.ca.gov is the single most effective escalation for California members — the DMHC has direct enforcement authority over Kaiser’s HMO license. Call: 1-888-466-2219.
  5. File with your state’s insurance regulator for members in Colorado, Georgia, Hawaii, Maryland, Virginia, DC, Oregon, and Washington State.
